Program Funded: Elder Services
Program Goal: Seniors will express an improved quality of life and/or independence through physical and occupational therapy, exercises, medication monitoring, meals and family counseling.
- Statistic: 176 clients served.
Program Funded: Disaster Services
Program Goal: Families will receive timely shelter, food and other basic need services immediately following a disaster.
- Statistic: 147 individuals served.
Program Funded: School Readiness
Program Goal: Training and support for parents on literacy, numeracy and other skills and expectations for their children's kindergarten entry.
- Statistic: 337 families partcipated in training and were given at home materials to work with their children.
Program Funded: Healthy Choices
Program Goal: Increase fruit and vegetable consumption and decrease incidences of childhood obesity through training parents on healthy eating and exercises.
- Statistic: 234 families partcipated, 150 reporting healthier eating habits.
Program Funded: Health Care Institute
Program Goal: Train parents on how to take care of their children’s minor injuries and ailments to improve their children’s health, decrease medical expenses and emergency room visits and reduce school and work days missed.

Program Goal: Youth participate in activities and build relationships which enhance education, and mental and character development.

Program Funded: Teen Mentoring
Program Goal: High school youth, male and female, provided vocational (exploring) and vocational (venture) programs through adult and peer mentoring, community service, leadership, etc.
- Statistic: 1,116 youth participants in 49 programs.

Program Goal: Re-engage and return chronically truant and drop-out youth back to school.

Program Funded: Community Resource Outreach Team
Program Goal: Parents, child care providers and children will receive screening, technical assistance, problem solving and referral through a mobile outreach team.
- Statistic: 2,071 referrals made to community partners, with 1,579 hours of instruction.
Program Funded: Child Abuse Prevention
Program Goal: Parents and children learn and use effective, safe, nurturing family management techniques to handle conflict at home.
- Statistic: 4,924 hours of child and parent instruction.
Program Funded: Early Intervention
Program Goal: Children with developmental delays or at-risk of development delays will improve skills to the best of their abilities and families will receive information, training and support.

Program Funded: Business Employment Services
Program Goal:Disabled persons will receive job placements, wages and job retention through trial work experience and job placement/retention services with community employers.

Program Goal: Through training in employment skills, individuals increase wages and progress towards community employment.

Program Goal: Increase financial literacy and health through financial counseling, credit review and educational workshops and personal budgeting.

Program Goal: Girls increase their awareness and interest in science-related subjects.

Program Funded: Preventing Complications in Uninsured Diabetics
Program Goal: Diabetics receive comprehensive medical care to enhance wellness and prevent complications.

Program Goal: Low income/uninsured persons will receive free medical exams and treatment, urgent dental services, pharmacy, prescription, lab dietician and other educational services.

Program Funded: Increasing Access to Social Service and Counseling
Program Goal: Clients will receive an evaluation of coping skills and emotional needs, counseling and community referrals to promote healthy living.
- Statistic: 4,389 individuals screened, with 345 counseled and 86 participating in group sessions.
Program Funded: Healthy Brain, Healthy Life
Program Goal: Clients will learn strategies to prevent brain injuries and maintain/improve brain health.

Program Funded: Chronic Pain Resource Guide
Program Goal: Individuals living with chronic pain will have a comprehensive resource guide to assist them in accessing the appropriate, affordable and timely care.

Program Funded: Vocational Case Management
Program Goal: Work to assist individuals with serious and persistent mental illness with vocational and employment assessment, planning, skills development, education, job placement and retention services.
- Statistic: 270 people served.
Program Funded: Children's Summer Day Treatment
Program Goal: Students will reduce problem behaviors in school, home and community; and will have successful transition to school in the fall.

Program Goal: Area residents will increase their awareness and use of health and human services through a telephone resource and referral system.
- Statistic: 57,131 total calls from Benton and Franklin Counties, with 81% indicating 2-1-1 helped resolve their problem.
Program Funded: Senior Home Delivered Meals
Program Goal: Ensure adequate and proper nutrition among homebound and frail seniors age 60 or older with a daily warm and balanced meal.

Program Funded: Senior Home Delivered Meals Under Age 60
Program Goal: Through referrals from organizations such as hospital social workers, doctors, hospice and kidney centers, a daily warm and balanced meal is delivered to homebound and frail individuals to ensure adequate and proper nutrition.

Program Goal: Victims of sexual violence will improve mental health to help reduce reoccurring abuse incidents.
- Statistic: 7,276 services provided to 397 clients.

Program Goal: Children will learn skills, awareness and personal accountability, which reduces abuse.

Program Funded: Therapeutic- Respite Recreational Services
Program Goal: Family members of individuals with developmental disabilities will receive the training, support and respite to alleviate the burden of care.

Program Funded: Buddy Club Youth Services
Program Goal: Children with intellectual and developmental disabilities will be matched with kids without disabilities to promote understanding and friendship and to prevent bullying.

Program Goal: Increase employability for adults with disabilities by providing basic work, social and communication skills training.

Program Goal: Promote parent-child bonding and early childhood literacy through a book program for first time parents.

Program Goal: Educational workshops to increase parental and caregiver knowledge to create a nurturing, healthy and safe environment for their first-born.

Program Goal: Provide free food to area residents who have expressed a need to ensure the consumption of adequate and nutritious food.

Program Goal: Increase children’s readiness for kindergarten by teaching social, personal emotional and cognitive skills; as well as working with parents on health and nutrition.

Program Goal: Provide year-round child care services, after school programs, field trips, nutrition and exercise programs for working parents or parents who are going to school.
- Statistic: Care provided for 634 children.
Program Funded: YMCA Martin Luther King Center Outreach
Program Goal: Family activities, youth sports, homework center, gymnasium and game activities offered in the East Pasco neighborhood.
- Projected Statistic: 95 students served in homework center. 89% of participants in health/exercise activities showed progress towards health goals.
